To submit your new patient forms, we’ll just need a little info from you. To start, you’ll answer a few simple questions about your child’s medical and dental history. If you plan on using insurance, you’ll also need your plan details such as your member number and other information that can be found on your insurance card. Your child’s new patient forms also offer a chance to tell us about any other considerations or healthcare needs you think we should know about.
